Name Reservation Applications

Before creating a new entity or applying for a new Legal and or Doing Business As (DBA) name, the Entity/Employing Broker Name Reservation Request Form (LI-213) must be completed and submitted for review. Click here to download the form, fill out the required information, and then attach it below.

This form is not to be used for a Professional Corporations (PC) or Professional Limited Liability Company (PLLC).  A PC or PLLC is not a brokerage or employing broker.
